HC²’s passion, creative thinking, and innovative strategies to develop new infrastructure to address health equity and the social determinants is one of the reasons we continue to work with the team. HC² was a founding partner of Communities Lifting Communities, which is the Hospital Association of Southern California’s community improvement initiative working to advance significant systems change through a collective impact model.
