Gabriela’s Journey to Wellness

For more than 20 years, Gabriela battled diabetes, joint pain, anxiety, and depression—chronic conditions that kept her in bed instead of enjoying life.

“I wasn’t physically able to do anything,” she explains. “I didn’t have no energy. I felt tired all the time, and I couldn’t sleep. I was always fatigued and tense.”

Gabriela couldn’t engage in the activities she loved as she struggled with her condition. “I wasn’t able to enjoy the physical activities with my family due to my health condition, or enjoying things like I used to….”

Then she received a life-changing call from Jessica, introducing her to Kaweah Health’s Enhanced Care Management services. As a Health Net Medi-Cal member with chronic conditions, she could receive personalized help in navigating her complex health and social needs.

In a follow-up visit the next day, she felt heard and understood for the first time in 20 years.

The support she received changed her life, she says. “Jessica gave me information for diabetes classes and advice.”

Gabriela’s health improved tremendously in the Enhanced Care Management program. Her blood sugar, blood pressure, weight, and depression all decreased.
